Dixie Boys All Star Paperwork

I will be at the fields Monday night, June 20th starting at 6:30 to obtain signatures from parents for birth certificates and medical releases of Dixie Boy parents ONLY. The credentials meeting is this Tuesday morning for this team so it is imperative that we get the forms completed for me to take to the meeting. Don't forget you need a 5x7 of the team with all names listed on the back in order to put in my folder.
